Registration



Our flexible month to month program.  No contracts.  No join up or membership fees.  Discount if you sign up for multiple sessions.

You have a number of Registration and Payment Options:
1) Online Registration.  Go to the tab labeled "Online Registration."  Once you submit the registration  form, the payment form will appear so you can complete your registration.
2) You may download and print the registration and payment form, and fax them in to our office.
3) Send payment with your registration!  You may call with your credit card information to our office or purchase online.
3) Once we receive your registration and payment, we will call you to schedule your camp
evaluation (we take measurements, blood pressure, etc. at the beginning and at the end of camp).
 
NOTE: We cannot guarantee your space will be reserved if you do not supply us with registration and payment information.  If paying by check, Please make checks out to:
 "Premier Sports and Health."
